Application for funds
Guidelines:
Minimum balance of $3500 left in account
Maximum monies awarded to an individual or group is $500 in a 3 year period
Possible use of funds:
Questionnaire administration
Occupational therapy or Physiotherapy research foundations
Individual or group research projects
Procedure:
Applicant arranges to be place in the meeting agenda with chairman and fills out the application form
Applicant presents request for funds at committee meeting
Membership votes on request (according to committee's terms of reference)
If funding is approved, applicant receives a check from treasurer
Note: The committee may request an applicant to make an educational presentation to the committee. If such a request is made at the time of the vote than funding will be awarded after the obligation has been met
Application forms will be kept by the treasurer
