Application for funds

Guidelines:

• Minimum balance of $3500 left in account

• Maximum monies awarded to an individual or group is $500 in a 3 year period

Possible use of funds:

• Questionnaire administration

• Occupational therapy or Physiotherapy research foundations

• Individual or group research projects

Procedure:

• Applicant arranges to be place in the meeting agenda with chairman and fills out the application form

• Applicant presents request for funds at committee meeting

• Membership votes on request (according to committee's terms of reference)

• If funding is approved, applicant receives a check from treasurer

• Note: The committee may request an applicant to make an educational presentation to the committee. If such a request is made at the time of the vote than funding will be awarded after the obligation has been met

• Application forms will be kept by the treasurer
